How do I obtain a user id and password to access appropriate portal?
 

You need to fill-out the Access Request form in order to obtain access to the portal portion of our website. Click here in order to begin the process.

What do I do if we have a recordable injury?
 

Log-in with your unique user id and submit the First Notice of Injury Report. A member of our Claims Team will respond within 24-hours to the submission.

Our RESOLUTION Team encourages you to also download and print copies of the Supervisor's Incident Investigation Report (linked below). This form should be used in the field for on the spot incident intake. Please turn in the completed form in addition to the FROI.
