Submitting the First Report of Injury is the first and most important step in beginning the process. You can click here to log-in to submit the report. Please be sure to provide all required information. A member of our RESOLUTION Team will respond to you within 24-hours.

Our RESOLUTION Team encourages you to also download and print copies of the Supervisor's Incident Investigation Report (linked below). This form should be used in the field for on the spot incident intake. Please turn in the completed form in addition to the FROI.
